Our Work In Sierra Leone:

Partners In Health (PIH) began supporting the government of Sierra Leone in response to the Ebola epidemic in October 2014. Recognizing the large need for continued investments in the health system after Ebola, PIH is committed to long-term health system strengthening in support of the Ministry of Health and Sanitation (MOHS), guided by the same principles as our emergency response and our work around the world: public sector accompaniment, integrated and comprehensive services, social justice, nimbleness and flexibility, partnership, community ownership, and evidence-based interventions. In the aftermath of the epidemic, PIH is committed to supporting the delivery of comprehensive health services in Sierra Leone, from the community level to the health center and to referral hospitals.

Position Overview

PIH-Sierra Leone is supporting the Ministry of Health and Sanitation in the implementation of the five-year World Bank funded QEHSSSP health systems strengthening project that will implement a comprehensive Hub-and-Spoke approach in 5 districts in Sierra Leone through focused investments in staff, stuff, space, systems and nutrition.

Through the project, Partners in Health, in the leadership of Ministry of Health and Sanitation, will be supporting 3 health facilities in the district (Jojoima, Buedu and Bandajuma) through comprehensive health systems strengthening approaches to develop these sites as sites for comprehensive Reproductive, Maternal, Neonatal, Child Adolescent Health and Nutrition services. The positions will be based in one of the health facilities in Kailahun: Jojoima CHC, Buedu CHC or Yawei Bandajuma CHC

This is a responsible, supervisory position filled by someone who is a good communicator, organized, passionate and positive. This individual works within the health facilities in Kailahun.

Duties include coordinating patient referral and follow-up care with Community Health Workers, counseling patients in various service areas specially focusing on RMNCAH services and data collection in coordination with MEQ. This individual will work with the CHWs and/ or Peer Supervisors to ensure pregnant women and sick children are followed up in the community.

Essential Duties and Responsibilities:

  • Link newly identified pregnant women and sick children to the CHWs including follow up to ensure patients are visited by CHW as planned.
  • Receive referrals from Community Health Workers with proper paperwork; accompany these patients through the clinical process
  • Receive referrals from other Facility Based Officers with proper paperwork; help guide these patients through the clinical process
  • Organize and manage files of RMNCAH, HIV and TB patients, and other categories of patients
  • Ensure that referrals make it to the clinic and help arrange transport if needed; relay appropriate information back to chiefdom supervisor/ peer supervisors and CHWs using follow-up forms
  • Ensure that Chiefdom Coordinators, peer Supervisors, CHWs have heads up knowledge about patients for Comprehensive Care appointment
  • Ensure patients attend follow-up appointments in collaboration with the CHWs.
  • Coordinate with Community Health Workers and Supervisors to obtain more information about patients to enhance clinical care
  • Counsel recently diagnosed patients alongside the HIV/ TB clinical team
    • Acting as a main liaison between the facility and the CHW team, including keeping a list of all patients and their attached CHWs
    • Interview patients for completion of necessary MEQ forms
    • Work with Community Health Workers and Supervisors to advocate for the needs of our patients.

      Education and Work Experience Requirements

      • High School certificate or diploma in any field of studies.
      • Commitment to social justice and human rights.
      • Should be an exemplary, honest, trustworthy, and respectful.
      • Experience with data collection
      • Proficiency in MS. Excel, Word and PowerPoint
      • Should be interested in community health and development
      • Should be a good mobilizer and communicator
      • Must be at least 18 yearsโ€™ old
      • Must be residing in the community in which they are serving.
      • Ability to ride a bike with a valid license, even in challenging territory.
